Position Summary: Provides social work encounters, case management, short term interventions and referrals for clinic patients in both medical and dental practices. Functions as a case manager for specific clients, coordinating services and treatment for clients. Supports the clinic model of behavioral health embedded in the clinic setting.
Primary Responsibilities & Duties
Meets with patients/families during visits and assesses current functioning, development and behavioral screening results. Provides short term intervention, if needed.
Focuses on interventions, and screening/assessments for dental patients.
Completes dental orders and/or tasks for dental patients.
Completes appropriate referrals to partners internal and external of clinic.
Attends Medical and Dental Staff meetings.
Keeps patient records up to date with notes in electronic medical record.
Protects patient confidentiality guidelines.
Maintain the spirit of team work in the effort to provide a caring respectful environment for the patient, families and clinic staff.
Covers other clinic social workers when needed (during call-off, vacation, special events).
Assists with holiday programs for clinic patients.
Attends appropriate workshops and community meetings monthly.
Responsible for performing other duties as assigned.
Knowledge, Skills & Abilities:
Ability to interpret verbal and nonverbal behavior in order to easily communicate with patients and staff. Understanding of diverse population and their mental health needs. Ability to multitask. Knowledge of Microsoft Office (Word and Excel).
Master’s Degree with two years of related clinical experience or training.
LCSW or eligible to take LCSW exam within 6 months.
Bilingual in Spanish
