The Antimicrobial Stewardship Coordinator works with physicians, staff, and committees to optimize the care of patients by providing and supporting drug use and evaluation processes, quality improvement and safety initiatives, and medication therapy education while embracing the philosophies of pharmaceutical care and comprehensive medication therapy management. This individual provides pharmacist expertise in the field of antimicrobial stewardship and infectious disease for both the inpatient and ambulatory care environment. The Antimicrobial Stewardship Coordinator works in conjunction with pharmacy administration to identify, justify, develop, implement, evaluate, and refine clinical pharmacy services and educational programs for Marshfield Clinic Health System.
Minimum Required: Five years of clinical experience in infectious disease or completion of PGY1 residency and PGY2 residency in Infectious Disease.
Preferred/Optional: PGY2 residency in infectious diseases, completion of an Antimicrobial Stewardship Certification Program is also desirable. Experience as a member and leader of Antimicrobial Stewardship Programs.
ESSENTIAL JOB FUNCTIONS
Develops and maintains a working relationship with prescribers and other Health System employees to develop consensus around system needs as related to drug therapy.
Provides pharmacist expertise in the field of antimicrobial stewardship including a working knowledge of medical information systems and their design, development, and integration into clinical practice and drug policy to ensure policies and procedures are followed accordingly.
Develops, enforces and maintains policies and procedures that promote and enhance patient outcomes through safe, effective, cost-efficient and appropriate medication therapy use for patients receiving antimicrobials.
Coordinates programs designed to minimize medication errors, adverse drug reactions and medication misuse with regards to medication preparations in infectious disease through improved reporting, analysis and follow-up.
Coordinates and maintains the antimicrobial stewardship program to improve the health and wellbeing of the communities served.
Oversees daily prospective audit of antimicrobials to ensure accuracy.
Assists in development and maintenance of an antimicrobial formulary approved by Pharmacy and Therapeutics Committee to improve antimicrobial utilization
Collaborates with the infectious disease service to evaluate appropriate use of restricted antimicrobials.
Develops of guidelines, policies, and other monitoring and intervention strategies for antimicrobial use and streamlining prophylactic, empiric, and definitive antimicrobial therapy.
Compiles data and metrics on antimicrobial use to track effectiveness of interventions and overall program performance.
Reports data and findings to the Antimicrobial Stewardship, Infection Control and Pharmacy & Therapeutics Committees to further follow up and review.
Coordinates, facilitates and promotes education, training, competency assessment and performance improvement of pharmacists, pharmacy residents and other healthcare professionals in the appropriate use of antimicrobials to maintain well trained knowledgeable staff.
Serves as pharmacy consultant with Infectious Disease service to ensure information needed is provided in an accurate timely manner.
Enhances and maintains the medications terminologies and rules infrastructure required for effective clinical decision-support and antimicrobial medication-use across multiple systems to ensure success for the program
Oversees the integrity of the medication supply chain to identify, investigate, monitor, and communicate medication shortages related to antimicrobials.
Provides knowledge of new medications and best practice recommendations to ensure best processes and medications are utilized.
Evaluates medical and pharmacy literature to promote high quality care and evidence-based medicine.
Provides clinical pharmacy services to patients and health care providers to ensure optimal drug utilization in assigned area.
Participates in various committees to ensure recommendations are consistent with evidence based medicine and in line with applicable internal committees.
Regular attendance is required in order to carry out the essential functions of the position.
Reviews and meets ongoing competency requirements of the role to maintain the skills, knowledge and abilities to perform, within scope, role specific functions.
