The UWMC Medical Specialties - Cystic Fibrosis Program has an outstanding opportunity for a Program Coordinator (Full-Time, Day Shifts).
This position is responsible for excellent customer service to the community, patients and other team members. •Serves under the general direction of leadership and performs work using knowledge and experience •Independently, advises the local and national centers leadership teams, providers, patients, family members and staff regarding care protocols and guidelines. •Functions as expert for the Cystic Fibrosis program identifying process improvements, recommending solutions. •Monitors specific centers’ equipment budgets and coordinate purchases from various vendors, monitors inventory, processes deposits, receipts. •Provides support services such as, managing the physician leadership's schedule, communication with key national agency directors, as well as coordinating client appointments/ complex testing procedures for the Cystic Fibrosis program, plus managing participants long term center involvement.
Position requires extensive involvement with members, staff and physicians in carrying out the centers and Cystic Fibrosis activities, consistent with the goals and protocols.
The primary duties of the program coordinator for the Cystic Fibrosis Program are as follows: •Assesses processes, communicate observations, concerns and ideas to the leadership and treatment teams in a timely and professional manner to maintain high standards of patient care. •Assists physicians and staff maintain systems to safely manage patients, •Updates demographic information and scheduling required tests and appointments for patients and providers. •Develops correspondence to communicate common issues that involve compliance and continuity of care.
Supports the Cystic Fibrosis program activities: •Prepares and maintains directors’ schedules, as well as coordinates conferences/center site visits and other activities. •Accompanies patients and clients between service areas to facilitate continuity of care, in particular, testing, hospital admissions and center surveys. •Acts as program representative with foundations and patient groups to arrange centers’ functions and events. •Interacts with many outside agencies, both national and international, including Muscular Dystrophy Association, Amyotrophic Lateral Sclerosis and Pulmonary Hypertension Care Centers. •Interfaces with multiple home nursing care agencies and equipment vendors, such as Norco, Sound Oxygen Service, Apria Healthcare and Lincare Medical Supplies. •Works with leadership and staff, plus other departmental staff members on various projects. •Generates specific reports as requested by center physicians and nursing staff. •Assists RN with routine medication prior authorization paperwork, phone calls, etc.
Coordinates and schedule extensive diagnostic tests necessary for evaluation and coordinate continuum of care for patients: •Reports patient concerns to MD/RN regarding their medical, social, financial, psychological or cultural needs as it relates to the planning, personal comfort, or safety of the planned test or procedure. •Interacts with patient to discuss itinerary of the diagnostic tests, pre-test patient preparation instructions, and answer questions. •Orders interpreter and translation of correspondence as needed. •Coordinates scheduled diagnostic tests with clinic visit appointments when possible but, per protocol, does not delay radiology or laboratory testing based on clinic appointment availability. •Responsible for continuum of care steps preceding and following initial appointment: requesting and ensuring timely receipt of outside records using EHealth; test, procedure and appointment scheduling; outgoing referrals and coordination of care with providers’ offices, and completion of missing outside provider documentation.
Participates in the preparation and maintenance of the DATABASEs : •Ensures that required information on each patient and provider is entered into the centers’ databases to monitor clinical outcomes.
Manages files for the Cystic Fibrosis Program: •Coordinates the receipt of requirements: patient forms, medical records, lab and diagnostic results from UWMC, primary care physicians’ offices, specialty providers’ offices and other medical facilities. •Ensures these results are delivered to providers to recommend and coordinate next phase of care. •In preparation for patients visits, ensures recent lab work, radiology results, and pathology reports are completed and available. •Follows up to ensure the procedures are completed or rescheduled if necessary. •Notifies providers of unusual delays in the completion of exams.
Ability to effectively communicate in English, both verbally and in writing; computer knowledge including Microsoft Office, data entry experience, and proficiency in word processing;
Ability to work effectively in a multi-disciplinary team environment and excellent interpersonal skills and customer service skills with outside agencies.
Demonstrated skills and experience in coordinating patient complex appointment care or other applicable experience.
Must be a flexible multi-tasker who can prioritize on a daily basis to meet patient needs and program deadlines.
Superior telephone and service-oriented communication skills.
